Chronic Care Management FAQs
We understand you may still have questions about chronic disease management at Abundance Health Care. We encourage you to explore the frequently asked questions below for more information or contact us if you need further assistance.
Chronic disease management entails anything from medication management and the implementation of medical devices (like an insulin pump for partnering patients with diabetes), to support in creating lifestyle changes and changes to the partnering patient’s diet. Dr. Bruner-Hill offers this guidance and support as often as the patient needs, ensuring they feel confident in their health and quality of care each and every day.
It is not uncommon for our partnering patients to struggle with managing more than one chronic disease at a time. In fact, a large portion of the older adult population has two or more chronic conditions that they have to manage day-to-day on their own. Dr. Bruner-Hill takes into account your health in its entirety when creating the right treatment and management plan for you. She understands that you are more than just your symptoms and works with you on a deeper level to understand your concerns and address them properly.
Absolutely! Dr. Bruner-Hill prides herself on working across teams to provide patients with the highest-quality, comprehensive care they both need and deserve. She will get in contact with your specialists to ensure your treatment plan addresses all of your concerns and does not conflict with any current course of treatment you are taking.
